David Miramontes has served his community as a firefighter, EMT, ICU/ER/Trauma Nurse, and as an Emergency Medicine and EMS Physician since 1999.  He graduated from the Medical College of Ohio and then completed his residency in Emergency Medicine at St. Vincent Mercy Medical Center.  He has served as a Fire-EMS Medical Director in Northwest Ohio and was the Assistant Fire Chief/EMS Medical Director for the District of Columbia Fire and EMS Department in our nation’s capital, Washington, D.C., from 2011-2014. He was in the first class of physicians to be subspecialty Board Certified in Emergency Medical Services in 2013. He has been the Medical Director for the San Antonio Fire Department since 2014.  During his tenure, the SAFD has implemented countless initiatives to improve the care of the sickest prehospital patients. He is the lead physician for the SAFD Mobile Integrated Health care service lines to include Hospice, High Volume Utilizers, and Addiction Medicine.  Dr. Miramontes is also a national leader in the field of prehospital medicine, serving on the Board for the National Association of EMS Physicians and as a member of the US Metropolitan Urban EMS Medical Directors Consortium (EAGLES).

CJ Winckler started his career in medicine as a paramedic with multiple high-performing agencies in Texas.  He is a graduate of St. George’s Medical School and completed his emergency medicine residency at the University of Mississippi in Jackson. His EMS fellowship was completed with 91PORN and the San Antonio Fire Department. After the fellowship, he was chosen as the Deputy Medical Director for the San Antonio Fire Department, which he has held since 2017. He is board-certified in both Emergency Medicine and Emergency Medical Services.  In addition to daily work as a full-time deputy medical director, Dr. Winckler’s responsibilities with SAFD include medical special operations, dispatch protocols, quality improvement, behavioral health initiatives, and implementation of progressive new practices.  In addition to numerous awards for his work, he has recently earned the School of Health Professions’ 2022 George Kudolo Award for Research Excellence for his work on the prehospital use of whole blood to transfuse patients suffering hemorrhagic shock.

Stephen Harper is a graduate of the Uniformed Services University of the Health Sciences in Bethesda, Maryland.  He then completed his emergency medicine residency at Brooke Army Medical Center in San Antonio and worked for several years as an operational and emergency medicine physician in the Army. He completed an EMS Fellowship in 2016 at the Military EMS and Disaster Medicine Fellowship in San Antonio, working closely with the San Antonio Fire Department. After leaving Active Duty, he became a Deputy Medical Director for the San Antonio Fire Department in 2021. He is board-certified in both Emergency Medicine and Emergency Medical Services.  Dr. Harper’s responsibilities with SAFD include public health relations, tactical medicine, on-call documentation, and quality improvement review.  He continues to serve our great nation as an Emergency Medicine Physician in the US Army Reserves as a Lieutenant Colonel.
